New Patient Information
We appreciate your selection of Cornerstone Health Services to assist you in managing your family's health care. This page was designed to acquaint you with our practice and our office policies. We hope it contains the answers. We hope it contains the answers to many of the questions you may have.
Your health is your most important asset. We are here to help you achieve and maintain good health. By learning about you as an individual with a unique personal and family medical history, we enhance our ability to serve you.
We encourage you to discuss any aspect of your care with us and we will try to answer any questions you have which are not covered in this page. Please retain this page so you can refer to it when necessary. It would be advisable for you to note your patient account number so that you have it handy when you call for an appointment.
Outpatient Services
We offer Xrays, microscopic exams of specimens, urinalysis, EKG, pulmonary function testing, sugar testing, flexible sigmoidoscopy, and many other tests while you're in our office. We attempt to identify and begin treatment as soon as possible without any waiting. More extensive diagnostic testing at Romeo Plank Diagnostic Center, located on Romeo Plank, just south of 21 Mile Rd. in the St. John Medical Bldg
Office Hours
Open daily, Monday through Friday, with evening and Saturday moming appointments also available. Most appointments are made in advance with your own doctor by calling their office number. If you have an urgent problem that requires immediate attention, you will normally be seen the same day. If you are unable to keep your appointment, please call the office number in advance. Your time is valuable, and so is ours.
Messages
Calls are returned by the doctors after seeing patients, except in case of emergency. Please keep your phone line free if you are expecting his/her call.
Refills and Prescriptions
Prescriptions may be refilled by phone if you have been seen recently. Please call during regular office hours and have the name and dose of your medication and your pharmacy phone number ready. Please call several days before you run out of your medication. Be aware that many medications may require you to be seen by a physician before it is appropriate to write a new prescription.
Forms, Letters, etc.
Forms or letters you need generated by your doctor's office generally need about five working days for their completion. Some forms may require an additional fee.
In Case of Emergency
Go directly to the nearest emergency room.
Always give our name so we can be notified. No insurance plan pays for non-emergency visits to the emergency room. Some insurance plans require a doctor's pre-authorization for emergency room reimbursement.
